INSURANCE & PAYMENT INFORMATION
I am an in-network provider for Aetna, Cigna, First Choice Health, Kaiser, LifeWise, Premera Blue Cross/Blue Shield, & Regence. I can bill many other insurance companies as an out-of-network provider. If you are using out-of-network coverage (UHC, UBH, and other insurers), your payment is due in full at the time of service and I will provide you with an invoice to submit to your insurance company for reimbursement. It is your responsibility to check with your insurance company to confirm what your benefits are. Please inquire if you have deductible and if it has been met, confirm your co-pay or co-insurance, and inquire as to the number of sessions you are allotted each year.
Co-payments, co-insurance, or private payment is due at the time of service. I accept cash, check, or credit cards. Please contact me directly with any questions regarding fees and sliding scale availability. There is a $50 fee for appointments that are cancelled in less than 24 hrs and there is a $100 fee for appointments that are missed without any communication.
